* The Aged Care Assessment Team (ACAT) comprehensively assesses the care needs of older people who have complex care needs that cannot be met by the Commonwealth Home Support Program (CHSP).
GOC Care Home Care Package options include:
- Personal Care: Assistance with daily tasks such as grooming, bathing, and dressing to promote independence and dignity.
- Clinical Care: Our in-house clinical care professionals prioritise wound care, medication assistance, nursing assessments, insulin management, nursing welfare checks, dementia advice, catheter care and continence assessments.
- Allied Health Services: Access to a range of allied health professionals including in home physiotherapists, occupational therapists, personal training, podiatry, remedial massage and more according to your individual needs.
- Domestic Support: We can assist you with vacuuming, mopping, dusting, linen changes, hanging washing and meal preparation. Our goal is to make your life easier and offer a comfortable and nurturing environment to suit your individual needs.
- Social Support Individual (SSI): We assist you to access the community for shopping, paying bills, attending medical appointments, personalised outings e.g. Coffee shop visits or short drives to your favourite place.
- Social Support Group (SSG): Engage in meaningful social activities tailored to your interests, fostering connection and joy. We offer monthly bus trips with your peers and social fitness groups, such as Hydrotherapy, Fun-Fit Exercise Programs, Choirs and Centre-based Wellness programs in South Brisbane and the Gold Coast.
