Support at Home (SaH)
The Support at Home Program was introduced by the Australian Government on 1 November 2025, replacing the Home Care Packages Program and the Short-term Restorative Care Programme. It offers a simplified, coordinated, and flexible approach to care, helping you live independently at home for longer.
An aged care assessment* will determine your eligibility for ongoing or short-term Support at Home services. The Australian Government provides classified funding based on your assessed needs to cover the services you receive under the Support at Home Program, including:
- Eight funding classifications for ongoing services.
- Three short-term pathways for assistive technology, home modifications, restorative care, and end-of-life care.
- Four transitioned Home Care Package (HCP) classifications for those who have transitioned to Support at Home.
Support at Home participants may be required to make a contribution.
* The Aged Care Assessment Team (ACAT) comprehensively assesses the care needs of older people who have complex care needs that cannot be met by the Commonwealth Home Support Program (CHSP).

- Therapeutic services for Independent Living: Your holistic health is our priority. We provide a variety of therapeutic services designed to nurture your mind, body, and spirit — including acupuncture, chiropractic care, diversional therapy, remedial massage, art therapy, and osteopathy.
- Domestic Support: We aim to make daily life easier so you can continue living independently at home. Our team can assist with grocery shopping, meal preparation, vacuuming, mopping, dusting, laundry, and changing bed linen — ensuring your home remains clean, comfortable, and welcoming.

Social Support and Community Engagement: Staying connected to your community and culture is vital to your well-being. We offer both individual and group social support options tailored to your interests
- Individual Social Support: Assistance with digital technology, video calls with family and friends, or accompaniment to community and social events
- Group Social Support: Opportunities to enjoy group outings, Fun-Fit exercise programs, the GOC Care ‘Fones’ Men’s Choir, and Centre-based Wellness Programs in South Brisbane and the Gold Coast.
